Provide in-home visits to Hospice patients and their families at their place of residence. Manage caseload of 20-30 clients providing psych-social assessments, supportive counseling, facilitate inner exploration of spiritual and emotional issues associated with end-of-life issues, crisis intervention, organize family meetings, assistance with attaining basic needs, and other concerns and needs identified by the patient and/or Hospice Team. Organize and facilitate educational forums and in-services on end-of-life issues for staff development and community education. Social Worker on Palliative team that provides support, information, referral and guidance to patients and their families in community, acute care and hospice settings. Manage caseload on consultation basis. Assessment of treatment and services for clients and their families on Palliative Care Program. Responsible for leading, planning, developing and implementing new and redesigned systems, policy and processes related to Medical Assistance in Dying (MAiD) services for all of Fraser Health Authority. Serve as a central contact, coordinates services provided by physicians, site leads and other health care providers and ensures continuous improvement in accessing timely, appropriate information and services to the public. Identify opportunities and recommends strategies for system improvement and leads and participates in meetings with key stakeholders to discuss the individual needs of clients seeking MAID and arranging the required support/services. Leads various projects related to medical assistance in dying; coordinates initiatives, prepares summaries and updates regarding project status, identifies issues, and recommends solutions.

Casual Social Worker providing counseling and advocacy for patients and their families who are receiving care at provincial Cancer Agency. Liaise with other health care professionals to provide optimum care, crisis intervention, adaptational counseling, provision of grief and bereavement support, staff debriefings regarding challenging cases or personal triggers related to oncology work.
