I accurately code ancillary and emergency department charts for Lexicode. I ensure all orders and reports are available and correct to code each account. I also add/change modifiers to clear any edits on the account prior to completing it. Once all orders/reports have been precisely read, the correct diagnosis codes are assigned, any modifiers needed are added, and then the chart is completed and finalized.

At Peak Health Solutions I was an Outpatient/Ambulatory remote coder. I coded for different facilities with different patient types, including: specialty ambulatory clinics, regular outpatient, ancillary, and other outpatient types as well. There was a big variety of specialty clinics that I coded daily, such as Cardiology, Urology, Orthopedics, Vascular, Dermatology, Audiology, Allergy/Immunology, Cardiothoracic, Neurology, Pathology, Rheumatology, Hematology, Endocrinology, Sports Medicine, Internal Medicine, Ophthalmology, etc.. I assigned the correct diagnosis codes, procedure codes, and I also would add/correct any modifiers needed to complete and finalize each chart.

I worked as a Remote Coder OP II with Conifer doing many types of outpatient coding. My responsibilities included reading and interpreting medical charts and assigning correct diagnosis and procedure codes for each chart. I also was responsible for 3M edits, modifiers, and E/M levels as well. I was assigned to code different types of outpatient accounts, which included: Regular Outpatient (with all types of diagnostic scans), Ancillary, Series, Hyperbarics, Sleep Studies, Oncology, and the Emergency Department which I coded on a regular basis, and worked as the primary coder for the Series and Hyperbarics accounts each month. Once the patient accounts in the Series and Hyperbaric types were released, they were time sensitive and always needed to be completed within the appropriate time frame, in which I made sure happened by the deadline. I also have experience in dual coding and have been fully trained in both ICD-9 and ICD-10 coding both, diagnoses’ and procedures. I completed and passed the appropriate education courses for that experience given by my employer. My production and accuracy were also held to high standards and I maintained those standards very well and almost perfect, in most areas. I was also asked to train the new employees who were hired to code at our facility. The training required me to instruct them on navigating the programs used and how to find all of the codes necessary to complete the chart; it also entailed me reviewing a certain number of their charts prior to them completing it to billing to ensure there were no errors or missing diagnosis or procedure codes.
