*Schedule home visits, post hospital and post fall visits*60 day supervisory visits including obtaining consumer vital signs*Supervise home health aides (HHA) and home health attendants (HA)*Generate care plans for consumers and go over it with HA/HHAs*Update care plans as needed per consumer’s health status/needs*Communicate with Supports coordinators regarding referrals, health changes, suggest an increasement in hours for consumers who need it most*Generate 485s (Plan of Care) and fax to physician to sign *Communicate with doctors about 485, missing signatures, consumer health changes, etc.* Open medicaid and medicare cases and import information into PRN and CradleMRx databases * Generate EIM reports and submit to HCSIS database* Submit initial and discharge OASIS to the state * Skill assess HA, HHA, LPNs, RNs Skilled Nursing (February 2015- 2017)* Cover cases for the scheduling department when no other nurse is available* On-call nurse that is called during after hours and the weekends when a problem arises, and/or a care plan needs to be read over for a cover aide. * Covered cases involving vent/trach care, wound care and administering insulin coverage to consumers

For example, I also served hot food and I also worked at the DIY station where I would assemble the station and make tacos, smoothies, hot drinks, etc.
