•Evaluated, developed individualized treatment plans, and implemented client-centered interventions to individuals with diagnoses including Parkinson’s, stroke, amputations, traumatic brain injuries, and spinal cord injuries to maximize functional independence.•Monitored and documented client progress, contributed to interdisciplinary team meetings, and constructed discharge summaries. •Provided education to caregivers regarding disease progression, adaptive techniques, and injury prevention to ensure safety upon discharge and reduce caregiver burden.
